Welcome to Kids Place Therapy Services! We are eager to work with you to help your child reach his/her potential. General information about how to get started is provided below.

We are an in-network provider for Blue Cross Blue Shield of Illinois PPO, Blue Cross Blue Choice and United Health Care.  We accepted other insurance plans as an out of network provider and private pay.  Additionally, we provide services through Early Intervention, an agency run through the state of Illinois, that provides services for children birth to three years of age.  Pre-authorization and filing of insurance claims are services we provide.

Upon contacting Kids Place Therapy Services, we will be happy to speak with you about your child and answer any questions you may have.  In order to schedule an initial evaluation, your insurance information is required, if applicable, to verify benefit coverage and obtain any preauthorization required.

Contact your child’s doctor to request a prescription stating “evaluation and treatment” to bring to the initial evaluation.  A doctor’s prescription is required in order to evaluate a child for therapy services.

In addition, please print and complete the intake questionnaire, patient registration form and patient services agreement below to bring to your first appointment. The HIPPA notice of privacy practices is for your review only. Completed forms may be faxed to 847-378-4615 prior to your appointment.
