Kids Place Therapy Services (KPTS) provides skilled therapy in a safe, welcoming environment for children and families. At KPTS we provide treatment for children with a wide variety of diagnoses including autism, sensory processing disorder and developmental delays. We are committed to providing comprehensive, multi-disciplinary care in a child-centered environment where children can achieve their goals and reach their full potential.

Welcome to Kids Place Therapy Services! We are eager to work with you to help your child reach his/her potential. General information about how to get started is provided below.

Insurance Plans

We are an in-network provider for Blue Cross Blue Shield of Illinois PPO. We accept other insurance plans, including United Health Care, Aetna, Cigna and Tricare, and private pay. Additionally, we provide services through Early Intervention, an agency run through the state of Illinois, that provides services for children birth to three years of age. Pre-authorization and filing of insurance claims are services we provide.

Setting up an Evaluation

Upon contacting Kids Place Therapy Services, we will be happy to speak with you about your child and answer any questions you may have. In order to schedule an initial evaluation, your insurance information is required, if applicable, to verify benefit coverage and obtain any preauthorization required. In addition to a copy of your insurance card, the initial paperwork is required to be completed and submitted. Please download and fill out the following three forms:

Prior to scheduling an evaluation we require the following documents:

  • A signed prescription from your child’s doctor dated within the last 6 months noting “Occupational Therapy Evaluation and Treatment”.
  • Completed forms from above
  • A copy of your insurance card (front and back)   *May be provided at initial evaluation

We will use your insurance information to verify benefit coverage and obtain preauthorization.

Meeting and Completing the Evaluation

During the evaluation we will ask questions based on your submitted information and complete assessments to better understand your child. We focus on minimizing your child’s stress. Many children report that the evaluation was fun!

We will discuss our impressions and therapy recommendations at the end of the evaluation. A final report will be mailed or provided to you.
